FERPA Release Form (Student Consent to Disclose Educational Records)

Overview

These are instructions for students to complete or update their FERPA release (student record disclosure form). This form is also called the Student Consent to Disclosure of Educational Records Form. Please note, this form does NOT cover medical records. For medical record releases, students should contact the health center about HIPAA. For more details, see Villanova University's FERPA Policy.   

Requirements

You must be an active student at Villanova University with access to Villanova NOVASIS and Softdocs forms.   If you are a student and cannot access either FERPA Release form, please contact registrar@villanova.edu from your Villanova student email address (i.e. username@villanova.edu).

Instructions

There are two formats available for the FERPA release form. Please click on the tabs below to view instructions based upon when you are submitting (new first time/making changes). 
Do not submit more than one form, unless you are adding, removing or changing the details for a previously submitted disclosure.

New Student Form - NOVASIS  

New students or those completing a FERPA release form for the first time at Villanova should use the NOVASIS form. If you have already completed it, NOVASIS will direct you to the Softdocs Etrieve Form (instructions below) to update your FERPA release.

Go to mynova and search for FERPA.   From the resulting task items, click on the task card labeled Student Consent to Disclosure of Educational Records (FERPA Release) - New Student NOVASIS Form.

If this is the first time you are completing the disclosure form you will be taken to this NOVASIS page.   (Otherwise you will be directed to the Softdocs Etrieve form (instructions below) to submit changes). By submitting this form, you consent to disclosure to the designated persons on the form.

If you are consenting to release of your educational records to a 3rd party, click the I Consent box and indicate the person(s) and their relationship to you.  Then, click Submit to submit the the form. 

If you DO NOT Consent to release of your educational records to a 3rd Party, click the I DO NOT consent box and then click Submit.

Once the NOVASIS form is submitted, your student record will automatically be updated with the notation that consent has/has not been provided to disclose information regarding your educational records to any designated parties.

Changes to FERPA Release - Softdocs Etrieve Form

If you want to update your FERPA release contacts or have trouble accessing the NOVASIS form (above), please complete the Softdocs Etrieve Form.

You can find this form by going to mynova and searching for FERPA.  From the resulting task items, click on the task card labeled Villanova University Student Record Disclosure Form - Student Softdocs Etrieve Form (alternative to NOVASIS).

You will be taken to the Etrieve form which requires electronic signature.   Complete the items requested on the form.  Once submitted it will route to the Registrar's Office where staff will update your FERPA release information on your student record.  Please wait for confirmation that the FERPA release notation has been updated.

Complete the top of the form with the Date, Your Villanova Number and Name should pre-populate on the form.

Check whether you are adding or removing persons to your disclosure.   Note: If you are replacing a person, you will have to submit two forms - one to remove the previous and one to add the new person.

Enter the name and relationship of the person(s) you wish to add or remove.

In the Signature section, click on the pen tip icon to open the signature item.

To Sign by drawing, click on the Draw Icon and use your mouse to sign your name.  Make sure to click on the legal consent box to check your agreement that this is a legal signature and then click on Create Signature.

To Sign by typing, click on the Type Icon and type your full name in the text box to create a signature.  Make sure to click on the legal consent box to check your agreement that this is a legal signature and then click on Create Signature.

 

Submit Signed Form.

Click the Submit button to send the form to the Office of the Registrar for processing.
